Case study: Preventing pressure ulcers

An 85-year-old patient was admitted last night after surgical repair of a right hip fracture. She is nonweight bearing and has pain when repositioned. The patient is on a schedule for changes in position every two hours and uses a high-density foam mattress. Will the patient's heels be properly protected from pressure-ulcer development with these interventions?

Answer: No. Although a pressure-reducing mattress and repositioning will help, the heels need further off-loading. There is a very high incidence of pressure-ulcer development after fractured hips and it is difficult to redistribute pressure on heels. As such, elevate the heels on pillows or provide adaptive equipment to off-load the heels.
