In 2002, the American Health Care Association estimated a 71% annual turnover rate among certified nursing assistants, who provide the majority of direct care for nursing home residents. To improve nursing home culture and care in California, the Culture Change Caravan-a program featuring training workshops for long-term care staff-recently embarked on a week-long tour. A coalition of federal and state agencies and healthcare organizations created the caravan, which stopped in five cities in the state. The program offers retention strategies to administrators and training for staff on several important issues, such as reducing pressure ulcers and restraint use, and improving organizational culture. Lumetra, Medicare's quality improvement organization for California, sponsored the caravan's workshops. Nursing home professionals from facilities throughout the state earned continuing education credits for participating.
