The Plan-Do-Check-Act (PDCA) model is a simple yet powerful tool for process improvement. This method has been modified over the years and originated within the manufacturing industry. The PDCA model has been used more recently in the healthcare industry and has been adapted by the Institute for Healthcare Improvement as a successful tool in achieving and sustaining clinical and patient safety improvement. The method begins with three fundamental questions:

  • What are we trying to improve?
  • How will we know that we had improvement?
  • What changes will be made to lead to improvement?
