A room filled with patient safety horrors helped educate nurses about what to watch for during a training exercise at Beebe Medical Center in Lewes, DE.

Nurses had five minutes to identify as many errors as possible in the room, which was designed to make participants more aware of infection prevention, patient identification, fall prevention, medication safety, and safety risk assessments.

Errors included expirations on medications, missing stoppers on a walker, and a bed that was unlocked.
