A new technology program has allowed nurses to tend to veteran patients from remote locations, shortening hospitalizations and providing team-centered care.

Care Coordination Home Telehealth, provided by the United States Department of Veteran Affairs, allows nurses to monitor a patient's vital information from home (such as the patient's oxygen level and blood pressure). About 30,000 veterans nationwide are using the technology.

Through the program, patients are supplied with an electronic monitor and are given instructions on basic procedures, such as checking blood sugar. The information is then sent to the hospital's computer. The system works over a basic telephone line, and the system can spot any irregularities, including fatigue or sudden weight fluctuation.
