Physicians requesting a leave of absence should state the reason motivating his or her request (e.g. to attend an education program, participate in military maneuver, etc.) and how long the leave will last. If the leave was not for an extended period-less than three months-the practitioner should prepare a summary of activities in which he or she will participated in during the leave. If a practitioner's appointment expired during the leave of absence, he or she must complete the full reappointment application.
