Prevent lawsuits by avoiding medical record red flags

As the number of hospitalists increase so does the number of patients under their care. As such, the chance for lawsuits increases as well, according to an article in The Hospitalist.
Hospitalists need to create legally defensible medical records by avoiding red flags that plantiffs’ attorneys often look for, such as:
- Illegibility
- Omissions of date and time
- Criticism of other providers
- Vague terminology
- Abbreviations
- Delayed entries
- Inconsistencies
- Corrections
- Opinions about the patient
