Admission is the best time to start a diabetes discharge plan

According to Hospitalist News Digital Network, the best time for physicians to start drafting a discharge plan for diabetic patients is when patients are admitted. This additional time allows hospitalists to determine social or financial factors that might get in the way of the patient complying with the discharge plan. For example, if the patient has little help at home or has trouble getting to the pharmacy, the physician can take these challenges into account and arrange for education and social services.
