AHA joins SHM?s Project BOOST

The American Health Association (AHA) senior vice president and Health Research & Educational Trust (HRET) president Maulik Joshi last week announced AHA’s participation in Better Outcomes for Older Adults through Safe Transitions, otherwise known as Project BOOST.
Spearheaded by the Society of Hospital Medicine, with support from the John A. Hartford Foundation, Project BOOST is aimed at improving the discharge process for at-risk elderly patients.
HRET’s recent “Health Care Leader Action Guide to Reduce Readmissions” is also available online.
