I wrote last month about the difficulties that hospitalists experience in satisfying their patients. I was involved in a case recently that brought to mind some of the ways in which physicians cause their own difficulties. A middle-aged woman was transferred to our hospital from a smaller facility for evaluation of diarrhea and abdominal pain. Her problem began a month earlier, and she had been admitted to another hospital for evaluation. The workup was inconclusive, and she was discharged to a nursing home in the area with instructions to encourage eating, but she was unable to do so because of pain. She was transferred to a different hospital, and then sent to my hospital.
