Lessons learned: VA quality concerns trigger hospitals to polish peer review practices

Additionally, investigations revealed that the brachytherapy program at the Philadelphia VA lacked a thorough and consistent peer review process.
The brachytherapy program was repeatedly green lighted by The Joint Commission and the Nuclear Regulatory Commission. It is not specifically known why those organization’s didn’t uncover more problems, highlighting the need for hospitals to carefully monitor each program’s and each physician’s performance.
Below are some tips to help your organization polish its peer review processes and ensure system inefficiencies or physician competency issues don’t escalate to the point at which harm caused to patients receives national attention.
