Nocturnists: The debate continues

Hospitalists continue to debate the overnight coverage model: do 24/7 programs actually improve patient outcomes, such as length of stay, mortality, readmission, patient satisfaction, or quality indicators? The December issue of ACP Hospitalist magazine (a publication of the American College of Physicians) outlines pros and cons of utilizing nocturnists.

Benefits include the ability of hospitalists to be present for nighttime admissions. Also, nocturnists frequently receive salary increases for their night work.

Disadvantages include recruitment, sleep deprivation for nocturnists, and the silo effect as a lifestyle. In addition, there is a lack of proven evidence of improved patient outcomes, according to the article.
