Hospitalist services for seniors

An article in The Daily Star (NY) discusses the benefits of hospitalist programs for elder patients. With more demands on primary care physicians, hospitalist programs could be an ideal service for seniors from admission to discharge. Since elder patients may have complicated medical histories, consider implementing a documentation program that asks your patients or their relatives to provide the hospital with allergy and medication information and PCP contact information.
