PM versus AM shifts: Discrepancy in care

The New England Journal of Medicine released a perspective article by David J. Shulkin, MD, president and CEO of Beth Israel Medical Center (NY) that highlights the differences between hospital operations during night and day. Shulkin found that the quality of inpatient care provided overnight versus during the day greatly differed, citing potential problems during nighttime hours:

  • Little or no senior managers present
  • Lower nurse-patient ratio
  • Fewer residents

Among the recommended solutions is the use of salaried hospitalists that are dedicated to off-hour coverage for inpatient care.
