Creating a palliative care team

Hospitalists are increasingly taking a role in palliative care to provide medical treatment and emotional support for the aging population. In addition to meeting Joint Commission accreditation standards, having a successful, systemic palliative care team may relieve stress on hospital staff and improve end-of-life care for patients. A palliative care team should consist of the following people:
- Hospitalist physician
- Patient’s primary care physician
- Nurse trained in palliative care
- Social worker
- Case manager
- Pharmacist
- Senior administrator
- Member of the clergy
