Iowa Healthcare Collaborative issues medication cards to improve medication safety

Hospitalist Leadership Connection, April 1, 2008

The Iowa Healthcare Collaborative (IHC) recently issued a card that may help improve communication between healthcare providers and patients, according to an AHA News article. Patients can use the wallet-sized card to list their prescription medications, over the counter medications and supplements, recent surgeries, and general health conditions.

Also listed on the card are questions patients should ask their healthcare providers, such as "What is the name of the medicine?" and "Do I need to stay away from food, drinks, other medicines, or activities while I take this medicine?" The card available for download at http://www.ihconline.org/toolkits/medicationbrochureandwalletcard.cfm.

The IHC began distributing 150,000 MedCards to the state's hospitals, physician's offices, and pharmacies in February.
