Deadline extension for Form CMS-1500 (12-90)

CMS announced last week that it has extended the deadline past the April 1, 2007, cutoff for changing from Form CMS-1500 (12-90) to CMS-1500 (08-05), which will accommodate the National Provider Identifier (NPI), according to a CMS press release. This will not affect the May 23 implementation of the NPI. Originally, CMS planned on making the Form CMS-1500 (08-05), which SNFs sometimes use to bill Medicare Part B, the only acceptable form after the April 1 deadline. But print vendors received improperly formatted forms from the National Uniform Claim Committee resulting in the printing of unusable documents, which do not comply with specifications. The correct form contains, in the upper right-hand corner a one-quarter inche gap between the tip of the red arrow above the vertically stacked word "CARRIER," according to the press release.
