Continuous observation of all residents for signs and symptoms that may indicate physical and mental changes is an important part of the resident's care plan. A CNA's observations may lead to changes or adjustments to the resident's course of treatment.
Resident observation is a constant, ongoing process that includes all residents-not just the residents to whom you are assigned. You should observe and report
- changes in the resident's physical condition or appearance
- changes in the resident's attitude, mood, or emotional condition
- complaints the resident expresses
