Tip of the week: Prepare a survey folder

It is very important for administrators to have a survey folder that is available at all times and is updated regularly. This folder should contain the important information surveyors may request upon entering the facility, such as:

  • Current census, including residents on medical or temporary leave
  • Copy of the actual work schedule for RNs, LPNs, and CNAs for the current work period
  • Facility floor plan
  • List of key personnel with their phone numbers, extensions, and locations in the building
  • Copy of admissions information and contracts
  • Copy of resident rights
  • List of residents admitted during the past month and a list of residents discharged during the past three months
  • List of residents 55 years of age and under
  • Copy of policies and procedures to prohibit and investigate issues of abuse
  • Contract/agreement with vendor that will provide emergency water
  • Copy of current activities calendar
  • Name of resident council president and his or her room number
  • Copy of facility license
