New tool for discharge planning released by CMS

A new CMS discharge planning tool was recently made available to beneficiaries and their caregivers, according to a letter issued by acting CMS Administrator, Kerry Weems. The tool, which involves the patients in the discharge process and helps prepare them to deal with their health condition in the home environment, consists of a checklist and information about available resources.
