Coding therapy on the MDS: Make sure minutes are coded accurately to avoid denials

The most recent report from the Comprehensive Error Rate Testing (CERT) program points to a slight uptick in the paid claims error rate for Medicare feefor-service skilled nursing facility (SNF) claims.
CERT analysis indicates that some of this increase can be attributed to insufficient documentation to support the rehabilitation therapy minutes coded in MDS 2.0 item P1b, therapies, the Medicare administrative contractor (MAC) Noridian Administrative Services, LLC, stated in a June 5 news bulletin.
