Working the resident assessment protocols (RAP) is a multistep process that requires the interdisciplinary team to employ critical thinking skills and analyze triggered problem areas.
Distinguish between the types of RAP triggers
“Many interdisciplinary team members don’t realize that there are different types of RAP triggers and that the type of trigger must be considered during the RAP process,” says Delores Galias, RN, RHIT, an independent health information management consultant in Santa Clarita, CA.
