The World Health Organization (WHO) says oseltamivir (Tamiflu)-resistant pandemic H1N1 cases may involve increased treatment doses for immunocompromised patients who have flu symptoms, according to a December 2 release.

Two recent clusters of Tamiflu-resistant H1N1 have been discovered in Wales, UK, and North Carolina. Both clusters involved patients whose immune systems were severely compromised or suppressed.

Staff members were not seen to contract the illness, suggesting that the resistant virus does not spread easily in otherwise healthy people. Experts agreed that severely immunocompromised patients need to be regarded as an especially vulnerable group and standard doses are likely insufficient. Doses may be increased and continued without interruption, although clinical judgment is necessary.
