How to score severity when conducting FMEAs

Editor's note: This is the second in a series of stories about failure modes and effects analysis (FMEA).

FMEA is a key tool to help identify and prioritize potential risks in systems, processes, and equipment. To do that successfully, you need to be able to lead your FMEA teams through the evaluation of the potential severity, occurrence, and detectability of each of the failure modes you identify.

This article will explore how to score severity, borrowing advice from Ken Rohde, author of the new book Failure Modes and Effects Analysis: Templates and Tools to Improve Patient Safety, published by HCPro, Inc., in Marblehead, MA. Rohde is also a senior consultant for The Greeley Company, a division of HCPro.
