Incident report
Long-Term Care Nursing Advisor, August 24, 2007
The incident report is used to alert administrative personnel to problems, gather data, and provide quality assurance information for studies and statistics designed to reduce incidents and injuries. Avoid referring to the incident report elsewhere in your documentation. Referring to it makes it part of the medical record.
