Coding tip: Recognize most common revenue codes

You use revenue codes (also known as "REV" codes) on UB claim forms and place them in field locator (FL) 42 on the claim.

REV codes are four-digits, with the first digit a leading zero ("0"). REV codes represent to the payer each charge or service provided by the ASC billed in FL 47. REV codes are listed in ascending numeric sequence on claims.

The REV codes most commonly used on ASC claims are the following:

  • 250-Pharmacy
  • 270-Supplies
  • 271-Non-sterile supplies
  • 272-Sterile supplies
  • 278-Implants
  • 279-Supplies
  • 320-X-rays/fluoroscopy
  • 360-Operating room time
  • 370-General anesthesia
  • 379-Other anesthesia
  • 490-Surgical procedures (used with CPT codes)
  • 710-PACU/recovery room time
