CMS confirms implementation date for UB-04 claim form
Ambulatory Surgery Reimbursement Update, May 15, 2007
CMS has confirmed that its strict deadline for the UB-04 implementation date will in fact be May 23, 2007. As of this date, CMS will no longer accept hard-copy claims, according to the CMS Web site.
From March 1 until May 22, providers may choose to use either the UB-92 or the UB-04 claim form, but after the implementation date, they must follow the new CMS guideline.
