PPV: Weigh the challenges when defining the legal health record

Although defining the legal health record might previously have been a fairly straightforward affair, the healthcare industry's transition to new technology has complicated the issue. Many organizations store patient records in a hybrid environment--on paper and in several, often incompatible, computer systems. This raises questions about how best to store, retrieve, and define the legal health record.
"There are no easy answers, and that's why people are struggling with it," says Mary D. Brandt, MBA, RHIA, CHE, CHPS, president of Brandt & Associates, a Bellaire, TX-based consulting firm.
