CMS-1500 deadline extended due to incorrectly formatted form
Ambulatory Surgery Reimbursement Update, April 3, 2007
CMS reported that the new CMS-1500 form, most commonly known as the 08-05, is incorrectly formatted and will be subject to rejection by carriers, according to FASA.
The CMS-1500 form was set to replace the old CMS-1500 (12-90) form starting April 1, 2007.
The deadline to use the CMS-1500 (12-90) form has now been extended until at least June 1 to compensate for this error. The old form cannot accommodate the National Provider Identifier, and applicants should note using a legacy number or UPIN on the 12-90 if they need to file claims while the issue is resolved.
Applicants can tell if they are using the erroneously formatted form if the tip of the red arrow at the top of the page above the word 'CARRIER' is close to or touching the top edge of the paper.
