Quality Improvement Monitor, March 9, 2007
Indiana, one of only two states that has a medical reporting system, had 77 reported medical errors last year, with 72 events taking place in hospitals and five happening in ambulatory surgery centers, according to Inside Indiana Business.
There were 21 incidents of a foreign object being left in the patient after surgery and nine incidents of surgery being performed on the wrong body part, according to a preliminary report on the errors. The final medical error reporting system report will be released in August.
