Preventing medication errors

To reduce the risk of medication errors, consider doing the following:

  • Always follow physician's orders and facility policies and procedures.
  • Monitor residents' laboratory test results carefully, and report abnormalities to a physician.
  • Consider potential risk factors, such as a decreased ability to swallow or neurological deficits, before administering medication.
  • Never document medications in advance.
  • Develop a care plan based on the resident's unique needs and individual risk factors.
