CT RFP to solicit Medicaid DM pilot proposals

The Connecticut Department of Social Services (DSS) is working to issue a request for proposal (RFP) this month for a series of disease management (DM) pilots that will focus on Medicaid patients with diabetes, asthma, high blood pressure, and other chronic illnesses. The pilot projects, which will be funded by $2 million authorized by Governor M. Jodi Rell ®, are expected to follow the model set by Connecticut's Easy Breathing asthma program. That program, which began in Hartford in 1998, is aimed at providing physicians with the tools they need to manage chronic illness, rather than at urging patients to improve their own behavior.
