Writing an appeals letter

Between patient care and daily documentation requirements, writing appeals letters is an added stress-albeit an important one-on your already tight work schedule.

However, appeals letter writing does not need to be a daunting task if you understand the process.

The opening portion of your appeals letter should cover the basics of the claim and verify the beneficiary information, including

  • denial decision date-use the date listed on the correspondence that notified your facility of the claim denial
  • beneficiary name-the name on your appeal letter should match the name listed on the claim and all other correspondence
  • Medicare number-the Medicare number should match the beneficiary's number listed on the claim correspondence
  • dates of services denied-the dates of service denied should mirror the information listed on the denial notification information
