PT clinic loses patient information

A representative from Northwoods Physical Therapy Clinic on Traverse City, MI, confirmed that the clinic lost a disk filled with patient information, reported WPBN-TV.
On July 21, the United States Postal Service told clinic officials that a disk the clinic had sent through the mail was missing.
The Northwoods clinic won't confirm how many patients the missing disk affects, only that it included patients who received treatment from 1998 through 2006.
The clinic told WPBN-TV that it provided public notice of the incident by posting an ad in a local newspaper warning of the lost disk, but would not comment on other actions it has taken to comply with federal privacy laws.
