Recent Sentinel Events Stemmed from Lab Errors

College of American Pathologists, press release, Dec: 1:"The CAP...[has] announced significant changes to the way its Laboratory Accreditation Program will inspect and monitor the performance of participating laboratories. "The CAP beleives these initiatives will enchance the consistency and effectiveness of our inspection process, strengthen monitoring of laboratory quality between inspections and monitor the performance of participating laboratories."

Baltimore Sun, Dec. 2: "In the wake of serious problems at two Maryland medical labs, the accrediting agency responsible for their oversight said yesterday that it will tighten rules for inspection and approval of thousands of laboratories around the world." Los Angeles Times, Dec. 2: "Lawsuits and complaints citing faulty lab operations are accumulating nationwide, naming some of the country's top medical centers. ...[a]llegations continue to mount. At least 10 cases are pending in state courts claiming harm from lab mistakes at Magee, a prominent regional hospital for women and part of the highly regarded University of Pittsburgh Medical Center."
