Study: Patients have much to say about adverse events

Engaging hospitalized patients in identifying medical errors and injuries can help enhance overall patient safety, according to a study published in the July Journal of General Internal Medicine.

Researchers reviewed the medical records of and conducted interviews with 228 patients at a Boston hospital. The interviews were done both during the stay and by telephone 10 days after discharge, and researchers asked about problems, mistakes, and injuries that occurred.

Of the 228 patients, 8% experienced 20 adverse events, with one considered serious. Eleven of the 20 events and four of 13 near misses were documented in the medical record, but none were noted in the hospital's incident reporting system, researchers found.
