Hospital worker convicted on four counts of identity theft

An Oregon deputy district attorney advised hundreds of medical record theft victims from Columbia Memorial Hospital (CMH) to check their credit reports, according to the Daily Astorian (OR).

Dawn Dazie Bennett, a former CMH worker, was convicted in April on four counts of identity theft after it was discovered she had 499 patient cards at her apartment and in storage.

The cards didn't include patients' Social Security numbers, but did have account numbers, patient names, medical record numbers, dates of birth, gender, dates of service, telephone numbers, ages, and the name of patients' attending physicians.

Investigators found no evidence that Bennett used the cards fraudulently, but the district attorney's office sent letters to the victims warning they should keep an eye out for any unauthorized activity.

The hospital initially sent out a letter when it recognized the breach.
