Physical restraint assessments

Long-Term Care Nursing Advisor, December 6, 2004

Prior to using a physical restraint, complete an assessment to determine whether a restraint is necessary and, if so, find the least restrictive device to treat the resident's medical symptom(s).

Prior to applying a restraint, assess the resident's

  • bed mobility
  • ability to transfer between positions
  • ability to transfer to and from a bed or chair
  • ability to stand and transfer to the toilet

Once you apply a restraint, reassess its use on a periodic basis. Use the Physical Restraint RAP to evaluate the appropriateness of restraint use.
