CMS to states: Adopt disease management programs and we'll match your costs

The Centers for Medicare & Medicaid Services (CMS) announced last week that it will match state costs of running "disease management" programs that target improved health and lower medical costs for chronic diseases such as diabetes, asthma, hypertension, and others.
"Disease management is an exciting opportunity to significantly improve the care delivered to Medicaid beneficiaries with chronic conditions," said Dennis Smith, acting administrator of CMS, in a press release. "It uses the best of managed care techniques of coordinating care that may not have been available in a fee-for service delievery system."
CMS defines disease management as medical interventions that work closely with the patients and their physicians to tackle treatment plans through diet, medical schedules, and self-care.
