Q: Can you clarify the reporting of dates on the plan of care for diagnosis onset and exacerbation?

A:    The instructions for completion of the plan of care, before the Centers for Medicare and Medicaid Services (CMS) deleted them from the CMS Internet-only Manuals in 2007, directed reporting of the date of onset or exacerbation on every plan of care.  Because those instructions no longer exist, an agency can determine its own course of action.  However, because the date helps to support the need for skilled homecare services, it remains a good idea to include it for sure on the initial plan of care and probably, every plan.
