California hospitals fined for preventable errors

California health officials last month slapped four hospitals with the maximum $25,000 fine for preventable errors that killed one patient, injured others, or created the potential for serious harm, according to the San Diego Union-Tribune.

The errors occurred at:

  • Pomerado Hospital in Poway, where a malfunctioning anesthesia machine used on three patients caused them to be partially awake during surgery
  • Scripps Green Hospital in La Jolla, where a patient fell to the floor during surgery because she was not restrained appropriately on a tilted operating table
  • Promise Hospital in San Diego, which allowed a nurse with a revoked license to treat patients
  • Sharp Grossmont Hospital in La Mesa, where a heart patient died after staff members failed to turn on a ventilator
