Treatment guidelines
Long-Term Care Nursing Advisor, July 11, 2008
Treatment typically consists of the following:
-
Gently clean the skin tear with normal saline or pH-neutral cleanser, according to facility policy
-
Allow the area to air dry or gently pat it dry
-
Assess and stage the skin tear. Measure size, if this is your facility policy
-
Approximate the flap, but avoid stretching the skin
-
Apply petroleum-based ointment, hydrogel, or a moist, nonadherent dressing, according to facility policy
-
Select a dressing based on wound size, characteristics, your assessment, and facility policy
