Order of documentation

Additionally, documentation should include the following items, in order:

  • Type of restraint (vest, belt, etc.)
  • Time it is to be applied (when up in chair, when in bed, at all times, etc.)
  • Reason for the restraint (avoid “safety”; list a concrete, resident-specific reason)
  • Release (such as every two hours, or according to facility policy)
