Wellmark implements medical home-based program

Des Moines–based Wellmark Blue Cross and Blue Shield of Iowa and Wellmark Health Plan of Iowa, Inc., are introducing a new suite of wellness and health management programs that focuses on encouraging members to establish a medical home and to take a more active role in managing their health.
Some health policy experts believe the concept of the medical home, in which care management services are provided through the primary care physician’s office, could be the most effective method for managing chronically ill patients’ care. The medical home concept has gained significant traction in the past year and will be the focus of a Medicare demonstration project beginning later this year.
