PPV: Is your staff using the UB-04 properly?

The new UB-04 billing form replaced the veteran UB-92 in May and now hospital billing/coding staff must use it to process all claims. According to Claudia Birkenshaw, MSA, CEO of Modern Management Muse, LLC, in Flint, MI, if you continue to use the UB-92 after this date, payers will deny your claims. Potential trouble spots to watch for during this period of transition include the following:
- Form locators 51 and 56, among others
- Physician national provider identifier numbers
- The present-on-admission field
- Changes to covered/noncovered/coinsurance/lifetime reserve days
