Current and former Medicare and PeachCare recipients in Georgia are at risk of identity theft after a CD containing the personal information of approximately 2.9 million beneficiaries disappeared, the Associated Press (AP) reports.

The CD, which contained addresses, birth dates, names, and Social Security numbers, disappeared in transit from Georgia to Maryland, Georgia health officials said April 10. Affiliated Computer Services, a vendor for the state, reported the breach.

Georgia is requiring the company to notify victims of the breach, according to the AP.
