Is it a HIPAA violation when the name of our radiology practice appears on patients' caller identification (ID)?

No. HIPAA does not require you to block the name of your radiology practice. In fact, if the caller ID identifies your practice as "private caller" or "blocked," it's likely that many patients would not pick up the phone.
However, clinics that treat specific diseases-such as AIDS-should limit the information provided by caller ID. For example, the Tenth Street AIDS Clinic should probably limit its display to "Tenth Street Clinic."
