May 23, 2007, is the date most providers must begin using their NPIs to identify themselves in standard transactions. It may seem a long way off, but there's a lot of work to do before then. And applying for an NPI is only one step.

Larger, complex providers are getting rid of thousands of old numbers, says Susan Miller, JD, independent consultant and chief operations officer at HealthTransactions.com. Without the proper preparation, your organization will not only be out of compliance, but it won't get paid, she says.

Your implementation plan should involve your health information management director, credentialing/contracts staff, CIO, CFO, privacy official, and information security officer (ISO), says Miller. And it should include the following steps:

  • Get buy-in from management
  • Look at organization's governance
  • Determine need for NPI subparts
  • Apply for NPI
  • Look at all contracts
  • Cross-map old identification numbers to NPIs
  • Determine systems impact
  • Test with payers
