How can we verify public officials’ identities when they request PHI?

You can rely on any of the following to verify identity when disclosing PHI to a public official or a public official's representative:

  • An in-person request accompanied by an agency identification badge, official credentials, or other proof of government status
  • A written request on appropriate government letterhead
  • A written statement on appropriate government letterhead that a representative is acting under the government's authority, or other documentation (e.g., a contract for services, memorandum of understanding, or purchase order) that establishes that the person is acting on behalf of the public official
