What records are not part of the designated record set?

Only documents containing medical records and billing information about an individual that are used to make decisions about an individual are included in the designated record set.

For example, peer review files include protected health information about many patients. However, these files are not part of the designated record set because they are used only to improve general patient care, not to make individual decisions.
